The FDA has identified Avanos Medical’s recall its Cortrak*2 access device for use in the system. Starting on March 21, 2016, 629 devices were recalls in the U.S.
Cortrak*2 permits clinicians to insert medical feeding tubes inside the stomach or small bowel of patients that require nutrition.
Avanos Medical feeding tube Due to injuries and deaths due to injuries and deaths, the system was recall. Avanos Mediacal Cortrak 2 Nasogastric, also known as nasal, tube that isn't properly inserted could result in damage to the vocal chords or the trachea. This can lead to serious injury, or even death.
According to Avanos the number of incidents has been an estimated 60 injuries and 23 death in connection with this issue. There were several adverse events, such as respiratory failure, pneumothorax and perforation, pneumonia and pleural effusion.
Cortrak*2 will use this recall to update the guidelines for usage and intended use. https://www.simmonsandfletcher.com/blog/injuries-incorrect-feeding-tube-placement/ The users will have to verify the location of tubes based on institutional protocols, before they are able to deliver nutritional supplements.
Clinicians were advised to add the corrective notice for field use concerning the issue to their operating manual and to return the acknowledgement form that was included with the notification to Avanos. Avanos is hoping to give users updated labels.
