Avanos Recalls Cortrak*2 Over Potential Enteral Tube Misplacement

The FDA has identified Avanos Medical's recall of the Cortrak*2 user access system. 629 devices manufactured between 2016 to 2022 were recalled in the U.S., beginning on March 21.

Cortrak*2 helps clinicians place medical feed tubes on patients who require nutrition through the tube.

According to Avanos Safety Communication, there have been 53 deaths and 60 injuries in connection with this incident. Many adverse events have been reported, including respiratory failure, pneumonia and pneumothorax.

Cortrak*2 will use the recall to revise its instructions for usage and intended usage. It will advise users to check the placement of tubes by using protocols established by institutions prior to delivering nutrition.

Clinicians are also advised to attach the field-correction notice to the operating manual , and return the acknowledgement forms included with the notice to Avanos. The users will shortly receive an update to the labeling and confirmation that the tubes were placed in accordance with the institution's policies.
