Avanos Medical (U.S. https://peatix.com/user/13103655 Food and Drug Administration) has announced that it will be recalling Cortrak*2's Enteral access system. https://setiweb.ssl.berkeley.edu/beta/show_user.php?userid=10553107 The recall was announced in response to injuries sustained by patients and death from misplaced nasoenteric/nasogastric tubes. The recall affects 629 devices which were distributed in the United States.
Avanos has launched the Cortrak*2 Entraal Access System to help with the installation of a medical-feeding tube. However, patients could suffer from damage to their vocal cords or lung if the tube is not properly placed. Avanos Mediacal Cortrak 2 Avanos Medical released a recall statement stating that there had been a total of 60 accidents and 23 deaths resulting from misplacement, in 2015, due to the CORTRAK*2 Access System. These injuries include respiratory failure pneumothorax (collapsed lung) as well as perforation (a hole in the lung's wall, esophagus or the bowel), pneumonia, and pleural effusion (excess fluid in the chest cavity and the lungs).

The Avanos warning is the second caution the FDA issued in 2022 concerning tube feeding through the stomach. In February the FDA issued a warning for parents and healthcare providers regarding the risk of strangulation when children are fed via feeding tubes. Cortrak 2 eternal access system Two deaths of children from strangulation were reported in 2021. due to a tubing system that was wrapped around a neck of a patient.
