In this sheet there are all the different events I was scheduled to go to for health equity. All fellow coordinators, site managers, and points of contact know that the Chinese Community Center will be at these events under my name (Rhyan Ooi). However, I have had to cancel all event slots and attendances due to not being in this position anymore. Please contact each event host and let them know that you will be attending (optional). These events are meant to streamline your health equity process to make it much easier. Each event is predicted to have over 1000+ attendees.
The process is simple, explain to individuals that the survey is from the Houston Health Department and is meant to record vaccine demographics. Answer questions from others as asked. Then, proceed to ask if they would like to fill out a survey (the vaccine survey) for a chance to win a gift card (amount of your choice). After they complete the survey, give them a raffle ticket and tell them to put their name and number on the one you keep. Draw the raffle every hour or so depending on the length of the event.
