Avanos Medical pulled Cortrak*2 EAS off service following reports of injuries and deaths caused by tube missing.
The US Food and Drug Administration (FDA), has declared the recall of Avanos Medical of Cortrak*2 Ental Access System, (EAS) as an class I recall.
The most serious of the three kinds is the Class I remember.
Avanos Medical pulled Cortrak* 2 EAS off service following death and injuries of patients due to misplacements of nasogastric tubes.
The device was created to aid trained health professionals in putting medical feed tubes for patients.

The company has recalled 629 devices distributed throughout the US between January 2016 and January 2022.
The company also said in a recall notice that there have been sixty injuries and 23 people who died since 2015 due to the feed tubes that were not properly placed when using the Cortrak* 2 EAS.
In the wake of the recall, Avanos Medical will update the labeling of the device. This will include changing the instructions as well as the intended uses for Cortrak* 2 EAS.
The updated guidelines instruct users to ensure that the tube has been set up according to the protocol of the institution before usage.
