Notes

Writer was called, resident was found on the floor lying on his back. Resident is alert and oriented to place and name. Denies hitting his head and denies pain at first. After 20 mins, resident complains of pain on his right arm and right side of the body. Writer administered 650mg tylenol. Pupils are equal and reactive to light. Both legs are equal and not externally rotated.

Writer left a message for Dr. Janz about resident's extremely low BP and weak state.

Writer tried to give resident fluids but resident only took a sip of water.

Three staff helped resident get him to a standing position and to his walker, where his legs were weak so he sat on his walker. He was leaning on his left side and seemed very lethargic after getting him to his walker. Both hands were very red, cap refill less than three seconds. Resident has ROMx4 strong on upper limbs but weaker on both legs. Resident seemed dizzy but was talking a lot for the first 5 mins. Writer and care staff then wheeled resident into his room and called another staff to get the sit to stand lift to transfer resident into bed. Resident was talking profanities and was a little frustrated. Was able to get resident into bed safely.

at 18:30 resident's colour is back to his normal colour, hands are also back to normal colour.
Resident awake and conscious, talking to writer he said he is comfortable and not in pain.
he is also back to his normal energy levels.
