Non-contrast-enhanced (NCE) magnetic resonance imaging (MRI) is widely used in pediatric epilepsies. The purpose of this study was to describe the role of NCE MRI in a group of children with seizure disorder and to discuss its utility in daily practice. We systematically included all consecutive children aged 0 to 18 years, who presented to our epilepsy centre between January 2012 and March 2015 and had a NCE MRI as part of their work-up (N = 115). We collected demographic data and other laboratory results; the primary diagnosis was based on EEG and clinical data, not on MRI. https://shariki.me/ Three experts evaluated the images using specific criteria. The most frequent abnormalities were structural (93/115; 80%) including periventricular nodular heterotopia (PNH, 39/113; 35%) and focal cortical dysplasias (FCD, 19/113; 17%), and vascular (30/113; 26%) and epileptogenic (24/113; 21%) abnormalities. Aetiology was identified in 21 patients (18%). Seizure types were analysed according to the diagnostic category: for 31 (67%) of the 49 idiopathic cases, NCE MRI had no impact on the final diagnosis, but there was false-positive diagnosis in four cases. In 31 (59%) of the 54 partial epilepsy cases, NCE MRI was misleading; in the other cases, there was false-negative diagnosis in one case. These results show the utility of NCE MRI when conventional MRI is contraindicated or unsuccessful.
