The FDA has identified Avanos Medical's recall of its Cortrak*2 useal access system. 629 devices manufactured between 2016 and 2022 were recalled, the first recall taking place on March 21.
Cortrak*2 helps clinicians place medical feeding tubes in the stomach or in the small bowel of patients who require nutrition via the tube.
The recall was brought on by injury and death reports from patients who suffered from nasocentric or nasogastric placement error. Cortrak 2 eternal access system This device is used for helping to place the tubes. An error in inserting a Nasogastric or Nasoenteric tube can cause serious injury or even death.
According to Avanos' safety communications there were 60 injuries and 23 deaths attributed to this incident. There were a variety of adverse events, including pneumothorax, respiratory failure and perforation as well as pleural effusion and pneumonia.
Avanos Medical feeding tube This recall is used to revise Cortrak*2's instructions on use, intended use and to remind users to verify tube placement according to institutional protocols before using the tube to provide nutrition.
