According to the U.S. Avanos Mediacal Cortrak 2 Food & Drug Administration (FDA), Avanos Medical is recalling the Cortrak*2 Enteral Access System because the misplacement of nasogastric or nasoente tubes has caused injury and death of patients. The recall currently comprises 629 devices distributed in the United States.

When a tube is placed incorrectly, it could result in damage to the vocal cords, lungs and the trachea. Avanos Medical feeding tube Avanos Medical released a recall communication stating that there has been a total of 60 accidents and 23 deaths resulting from the misplacement of a tube in 2015 due to the CORTRAK*2 Access System. Avanos Mediacal Cortrak 2 These injuries include respiratory failure, pneumothorax (collapsed lung) as well as perforation (a hole in the lung's wall, esophagus, or the bowel), pneumonia, and an effusion of the pleura (excess fluid in the chest cavity and the lungs).
Avanos Medical describes a plan to revise labeling in order to ensure proper placement of tubes for nasogastric and nasoenteric before it is used.
Avanos is the FDA's second warning about inserting feeding tubes. The FDA sent a warning to healthcare professionals and parents regarding the danger of strangulation when feeding tubes are used by children. Two instances were reported of strangulation among children who had a feeding tube tied around their necks.
