The Avanos MedicalCortrak*2 Enteral Access System is designed to help trained health care personnel place medical feeding tubes in the stomach or small bowel of patients who have to be fed through the tube. There have been 23 deaths reported to date due to feeding tubes in 2015.

Avanos Medical feeding tube The recall was used to make modifications to the device's labels, including instructions for usage and uses. The new labeling instructs users to verify that the tube has been set up according to the institution's protocol before using the tube for nutrition delivery.

According to Avanos Medical's recall communication there have been 60 cases of injuries and 23 deaths due to the incorrect placement of nasogastric feed tubes when using the CORTRAK* 2 Enteral Access System, since 2015. Pneumothorax, respiratory failure perforation (a hole, or hole in the stomach wall) and pneumonia were all reported.
Avanos Mediacal Cortrak 2 Avanos Medical issued a correction notice to those who purchased these devices between January 2021 to January 2022 on March 21 2022. The notice included these instructions:
Confirm placement of the nasogastric, nasoenteric and other tubes in accordance with the policies of the institution.
