STEP 1: Help him realise that girl, Lillie is a bad person. She dropped Alex for another guy and sounds manipulative. Plus, not being loyal to her girlfriend. HE NEEDS to stop talking to her. He may like her still without realising it, which is why he may not see ALL the red flags. He takes focus off other people and only focuses on how SHE treats him. Like, do other ppl compliment him?
STEP 2: Make him realise he is NOT an annoyance or burden to anyone. Ask him to ask ppl if they find him annoying.
STEP 3: Alex needs to know that just because one girl thought he wasn't good enough does NOT mean that the other 8 billion people will think the same. Everyone looks the way they look, and just because you're not pencil thin or don't have abs doesn't mean you're not beautiful. The fact he is a good person and cares for others already makes him beautiful. I cannot make him love himself, for the way he looks. He has to steer himself in the right direction. It IS possible to love yourself. You just need to believe, have faith. I know that is way easier said than done, which is why you need to steer yourself the right way, take it in steps.
