The Avanos Medical Cortrak*2 Ental Access System was created to assist health care professionals in placing medical feeding tubes in the small intestines of patients or stomachs. Since 2015, there has been 23 deaths linked to feeding tubes.

The updated instructions instruct users to ensure that the tube is placed according to their institution's protocol before making use of the tube for food delivery.
Patients may suffer serious injuries or even die if a nasogastric tube or nasoenteric tub is incorrectly inserted.

There were a variety of adverse events, including respiratory failure and pneumothorax (collapsed lung tissue) and perforation (a gap or opening in the esophagus's wall or the bowel), pneumonia, and the pleural effusion (excess liquid in the space between your lungs and chest cavity).
Avanos Medical issued on March 21, 2022, a field correction notice to its customers who were using the devices between January 20,21 and January 20,22. The notice contained the following instructions:
Verify the placement of nasogastric as well as Nasoenteric tubes in accordance with institutional policies.
Attach the appropriate note of correction to the field in the operator's guide.
Avanos Mediacal Cortrak 2 Avanos Med will ask that you return the acknowledgement sheet that was included in the email notice.
