The Avanos Medical Cortrak*2 access system is designed to allow trained health professionals to place medical feeding tubes in the stomach or the small intestine of patients that require nutrition. The feeding tube has been linked to 23 deaths since the year 2015.

Avanos Mediacal Cortrak 2 This recall is being used to make updates to the device's labeling. This includes the instructions for use and the intended use. The new instructions tell users to verify the positioning of the tube as per their institution’s protocols before making use of them to deliver nutritional supplements.
Avanos Mediacal Cortrak 2 Avanos Medical Incorrectly inserting a Nasogastric or Nasoenteric tube could cause harm to the vocal cords, lungs, or the trachea. This could lead to serious injury and even the death.
According to the Avanos Medical recall communication There have been 60 injuries and 23 deaths due to misplacement of nasogastric feeding tubes when using the CORTRAK* 2 Enteral Access System, since 2015. Affected events include respiratory failure, pneumothorax (collapsed lung) and perforation (a hole in the wall of the lung, esophagus, or bowel), pneumonia (a lung infection), and pleural effusion (excess fluid that has accumulated in the space between the lungs and the chest cavity).
Avanos Medical issued a correction notice to those who purchased these devices from January 2021 to January 2022, on March 21 2022. The notice included the following guidelines:
Confirm placements of nasogastric/nasoenteric tubes according to the institutional guidelines
Attach the correction notice from the field that explains the problem to the operator’s manual.
Avanos Med will ask you to return the acknowledgment sheet that was included in the email notice.
