According to the U.S. Food & Drug Administration (FDA), Avanos Medical recalls the Cortrak*2 Enteral Access System because the incorrect placement of nasogastric and nasoenteric tubes has resulted in accidents and deaths of patients. Avanos Mediacal Cortrak 2 The recall affects 629 devices that were sold in the United States.
Avanos has launched the Cortrak*2 Entraal Access System that assists with the installation of a medical-feeding tube. Patients may experience damage to their vocal chords or trachea, if the tube was placed improperly. The recall notification from Avanos Medical states that as of 2015, there have been 60 patient deaths and 23 injuries resulting from incorrect placement of nasogastric feed tubes using the CORTRAK*2 enteral access system. Avanos is the FDA's second warning regarding insertal feeding tubes. The FDA sent out a message to healthcare professionals and parents concerning the risk of strangulation in the event that feeding tubes are being used by children. Two deaths from child strangulation took place in 2021 because of tubes that were tied around the neck of a child.
