Avanos Medical Cortrak*2 Access System is designed for medical professionals who are trained to insert medical feeding tubes inside the stomach or small bowels of patients who require nutritional support. There have been numerous reports of patients suffering injuries and even deaths due to misalignment or misplacement of nasogastric, also known as the nasoenteric tubes. Avanos Medical They are employed to aid in their placement.
This recall is used to update the device’s labeling that include the directions to use. These updates require users to confirm the placement of the tube as per the guidelines of their institution prior to using the tube to distribute nutrients.
If you do not insert correctly a Nasogastric tube or Nasoenteric tube can cause damage to the vocal chords, lungs , or the trachea. This can lead to grave injury and possibly death.
Cortrak 2 eternal access system According to the Avanos Medical recall communication there were 60 injuries and 23 deaths of patients due to the misplacement of nasogastric feeding tubes while using the CORTRAK* 2 Enteral Access System, since 2015. The adverse events that were reported include respiratory failure and pneumothorax (collapsed lung) and perforation (a hole in the lung's wall, esophagus or bowel) as well as pneumonia (a lung infection), and the pleural effusion (excess fluid that has accumulated in the lung space and the chest cavity).

Place nasogastric or nasoenteric tubs in accordance with the institution's policies.
Attach the pertinent note of correction to the field in your operator's guide.
