Avanos Medical faces a Class I recall of its feeding tube system connected to 23 deaths in 2015.

The FDA has issued the most severe of warnings in relation to the recall of Avanos Medical’s Enteral Feeding Tube Positioning System.

Cortrak*2 Enteral Access System has an electromagnetic stylet as well as an external receiver. It also includes an LCD monitor. The system, when used together with the display monitor, allows live video of the medical feeding tube being placed in a patient's stomach.

Avanos Medical feeding tube Despite that goal, it has been linked to many injuries sustained by patients which led Avanos to issue a recall in January of all Cortrak*2 devices between January 2021 and January 2022, totalling nearly 630 devices first distributed between April 2016 until the beginning of the year.

The safety event is not considered to be an recall. Avanos has asked healthcare providers not to send the devices back to the manufacturer. But, they would like to make sure that they are using the devices.

If a feeding tube is placed incorrectly it could cause damage to the vocal cords, lungs or trachea and cause severe injury or death. In actual fact, Avanos has received reports of deaths of 23 and 60 injuries since 2015 the FDA stated, all of which were related to the the Cortrak*2 system to guide the installation of a feeding tube.

The FDA has reminded patients that they need to confirm the placement of nasogastric or nasoenteric tube according to their institutional policies in the March 21 field correction notification. Avanos has also asked users to add the note on safety to the operating manual for the system and confirm that they've received the update.

Avanos has announced that they will soon release a updated labeling for its device. It will also include instructions to indicate the location of the tube according to their facility's policies.

This is the second FDA warning on enteral feeding tubes this year. In February, the FDA issued a safety message to parents and healthcare providers about the potential for strangulation of children using feeding tubes.

Cortrak 2 eternal access system The notice followed two reports of death in 2021. In each instance the tubing system was discovered to accidentally wrap around the neck of a child under the age of two even though they were not being watched by the medical staff or caregivers.

"The FDA believes that death and serious injuries resulting caused by strangulation with tubing used for enteral feeding for children is uncommon.
