Avanos Medical Announces Recalls of the Cortrak*2 Entry Access System
Avanos Medical (U.S. Food & Drug Administration) has announced that it will be recalling Cortrak*2's Enteral Access System. The recall was initiated in response to injuries sustained by patients and deaths resulting from the misplacement of nasoenteric/n tubes. The recall affects 629 devices currently distributed across the United States.

Avanos Medical feeding tube Avanos has launched the Cortrak*2 Entraal Access System that assists with the installation of a medical-feeding tube. If a tube is inserted incorrectly, however the patient could suffer damage to their vocal cords, lungs or trachea. Avanos Medical feeding tube Avanos Medical recalls that there are 60 accidents, 23 deaths, and 23 illnesses that were caused by the misplacement or inadvertent insertion of a feeding tube for nasogastric patients using the CORTRAK*2 Entral Access System. The injuries have included respiratory failure pneumothorax (collapsed lung), perforation (a hole in the lung's wall, esophagus or bowel) as well as pneumonia. an effusion of the pleura (excess fluid in the chest cavity and the lungs).

Avanos is the FDA's second caution about inserting feeding tubes.
