Psychotic Dementia Treatment
A small but significant percentage of people with dementia develop psychotic symptoms. Symptoms include hallucinations, which are thoughts or images that don’t occur in the person’s real world; delusions, which are false beliefs; and aggression.

The incidence of dementia-related psychosis varies by the type of dementia a person has, with DLB and Parkinson’s disease dementia (PDD) having the highest rate. Hallucinations are the most common symptom in these patients and are more likely to be complex visual than auditory. These occurrences are often related to the Lewy body plaque deposits (Lewy bodies) on the brain that interfere with the brain’s chemical messaging system.

Other symptoms of psychosis in these two groups are anxiety and depression. dementia and wandering Patients with these symptoms also have a high burden of comorbidities and frailty, with a higher incidence of emergency department visits, oral anti-infective use, and infections following their psychosis diagnosis.

There is limited evidence that antipsychotic medications help with these symptoms in a dementia setting, so these drugs should be prescribed with care. It’s best to try other non-drug treatments before trying them, especially in older adults who are more susceptible to side effects of medication. For example, music and pet therapy have shown to be beneficial for those with psychosis. Restraints are also used to keep a person safe, but this approach should only be employed in an urgent situation. When a person is at risk of harming themselves or others, antipsychotic drugs can be administered, but only in the lowest dose possible.
