There are several ways to apply, including meeting with a caseworker, applying by phone, applying in our computer room, or mailing in an application. Which option would you prefer?

You have the right to apply for benefits today.
► You must sign an official application form to start the application process.
► Unless you sign an official application and complete an eligibility interview, you cannot receive benefits.
► If possible, you should wait to talk with a caseworker before signing an application.
►The caseworker can give you more information about the programs for which you may be eligible.
► If you are unable to stay to see a caseworker today, tell the receptionist immediately.
►Explain that you must leave, but that you want to sign an application.
►An appointment will be arranged for you to complete the application.
►You will be told the appointment date and time and every effort will be made to accommodate your schedule.
►However, repeated failures to keep interview appointments may result in your application being denied

►Getting an interpreter will not delay your application or benefits.
►This application may ask you to tell us the social security number and immigration status of everyone living in your household.
►If you do not have a social security number, we can help you get one.
►Social security numbers will NOT be used to report anyone to the Immigration and Naturalization Service (INS).
►You will still need to tell us about your family's income and answer the other questions on the application form.
►EMERGENCY MEDICAID
Anyone applying only for emergency Medicaid does not have to provide his or her social security number or information on his or ►her citizenship or immigration status.
