12,腓肠肌痉挛
腓肠肌痉挛，中医叫“转筋”，俗称“小腿抽筋”或“小 腿肚子转筋”。患者常常在睡眠中小腿肌肉突然抽掣拘挛、扭 转急痛，必须忍痛用力伸足，甚至下床挺立才能缓解。此症多 由气血不足，寒湿侵袭或局部肌肉过劳（如游泳、过度运动 等）所致。
承山

太阳之筋……结于胭，其别者，结于踹外……其病……胭挛[ ……”对于经筋病的治疗原则，经筋篇说：“治在燔针劫刺，

“以知为数”就是不论次数多少，症状消失即停止治疗；“以痛 为输”就是哪儿痛就在哪儿针刺。提醒大家注意的是，《灵枢公 惜字如金， 서귀포출장안마 12遍.这告诉我 们，凡是经筋病的治疗，都应该以局部取穴为主，所以《灵 枢•终始第九》说：“在筋守筋。” 서귀포출장마사지 素问•调经论篇》说：“病 在筋，调之筋《灵枢*四时气第十九》也说：1‘转筋于阳治 其阳，转筋于阴治其阴。”
综合以上论述f对于筋病的治疗，我们应该采取的方法 是：在取穴上，要以局部取穴为主，哪儿有病痛就在哪里取 穴，有经穴则取经穴，没有经穴则取阿是穴；在针灸方法上，

要结合温热刺激，因为物体的--般规律是热胀冷缩，阳气具有 温煦的作用，即“阳气者，柔则养筋”，可用火针，可用温针 灸，也可用针刺后加TDP等照射，还可以用热熨，如《素 问•血气形志篇》说：“病生于筋，治之以熨引。”学医重在明 理，妙闬在人；在针刺深度t，要直达筋部，既不能太浅，也 不能太深，这就是《内经》上反复强调的“刺筋者无伤肉…… 剌筋者无伤骨”的含义^
腓肠肌痉挛的部位il:好是承山穴 所在，所以临床上多刺承山穴，用温 补手法，或温针灸或加TDP等照射，
均有佳效，常1次见效或痊愈，一般 不会超过3次。
如治某男，55岁，因股外侧皮神 经炎来诊，经针刺治疗症状基本消 失。治疔过程中又诉说每夜小瞇肚抽 筋,影响睡眠，为针承山穴，加TDP 照射，只针1次，痉挛即止，半月 后，又偶有抽筋，再刺承山，痉挛又 止。
承山（BL 57)属足太阳膀胱经 穴，在腓肠肌两肌腹之间凹陷的顶端 处，见图28,
