Notes

1. I am allergic to brufen group medication it causes my urine to turn red and urine contains blood
2. Pain is located on the top of the right testical, i did notice the right side scrotum to be swollen. The lump of nerves on top of the right testical is the main center of pain and the pain passes along with the nerves going inside.
3. The pain was mild till afternoon yesterday (21jan). The agony started around 4 Pm (21 Jan)
4. Night 11:00 PM I was feeling very cold and mild headache along with rise in tempature. I did consume approx 90 ml of alcohal. I am not on any kind of medication.
5. I was not engaged in any kind of sexual activity for the last 7 days, although was struggling with day shift due to sudden change from night shift to day shift and was stressed out,exhausted while training the batch. I was unable to sleep more than 3 hours in the night since monday (18 Jan) as I would only feel sleepy around 5 - 6 am.
6. My first reaction to the pain was strain on any of the nerves or muscles and as suggested the remedy was a good night sleep. I am suffering acute pain while making any lower body movement.
7. There is no such history of this kind of pain in my family. I do not experienced any such incident. I do have a scar on my stomach which was due to appendix operation in my childhood. There is no pain in that region.
8. I am uncomfortable speaking about my ailment in general, hence the written version in case it gets severe.
9. My wife's number in 9632340293 in case I get hospitalized or you need to seek consent for any kind of medical procedure and I am unable to provide the consent due to any reason.
10. I would really appreciate it if the nature of my ailment is not disclosed to my colleagues and is kept to person who need to know and get involved in facilitating the treatment or cure.
11. I am unware of my blood group as I have not been hospitalized since childhood.
12. The first person to talk to my parents will be me and please do not notify anyone in my family other than my wife.
13. I have worked 15 months straight without taking any leaves or unscheduled leaves. I hope it is just a strain so that I can resume training the batch and contribute to making our process the best process in the whole company.
