臀部外伤出血、粘连、瘢痕形成；注射药物使梨状肌变性、纤维挛缩；髋臼后上部骨折移位、骨痂过大均可使坐骨神经在梨状肌处受压。此外，少数患者因坐骨神经出骨盆时行径变异，穿行于梨状肌内，但髋外旋时肌强力收缩，可使坐骨神经受到过大压力，长此以往产生坐骨神经慢性损伤。
2、治疗
（1）目的：舒筋通络、活血止痛，改善局部气血运行与代谢，促进炎症、水肿、淤血的吸收，解除对神经、血管的刺激和压迫，缓解或消除疼痛，使损伤组织恢复。
출장마사지 2）部位及取穴：环跳、承扶、风市、阳陵泉、委中、承山及臀部、坐骨神经分布区域等。
操作：
1、轻柔缓慢的滚法、按揉法从臀部至大腿后侧到小腿5-10遍。
2、弹拔法由臀部，腿内侧向外侧弹拔3-5遍，主要区域梨状肌处，时间稍久。
출장안마 、点揉穴位，力量轻柔缓慢。
출장 、直擦法从臀部至腿部5-10遍。

6、叩击法叩击臀部大腿后侧。
출장샵 、平卧位转腰运髋手法5-10次
（4）其它治疗：
1、辅助治疗牵引
2、针刺以上穴位，采用平补平泻手法，可加灸法
3、拔罐
