Executive summary
I propose to increase health literacy through comprehension in an older adult community center who solely speaks Spanish, through health presentation and wellness programs activities. My objective is to provide information to make aware of the health condition. Dispel any myths that are not helpful to health. Present other options where they can obtain information on chronic health conditions. I will focus on the importance on maintaining a normal blood pressure. and medication compliance. Noncompliance to management of their blood pressure and not taking their medication are the two most important condition that send the older adult to the hospital. I will gather the community center demographic information. I will be using two health literacy screening tools to assess base line. After obtaining my baseline information. I will also do pre and post survey after each presentation.








Shopp neighborhood was founded in 1980 and incorporated in 1981. Neighborhood Self help by older persons project,inc is a nonprofit multiservice agency , serving over 5,000 hispanic , african american, and West Indian Caribbean over the year. Their center office is located in Bronx community board district 2. There are now thirty five off-site service locations in almost every community board district in the bronx. Its funding partners are the NYC Department for the Aging , the New York state office of victim services office of the Bronx borough president, the NYC Department for the Aging , and New York state office for the aging. The purpose of the neighborhood shopp was to establish , develop and maintain a senior citizen multiservice agency which would serve people older than 60 and plus Bronx residents Their mission is “Helping Older Adults to help themselves.” SHOPP provides programs that empower older persons through knowledge , support , stimulation and motivation. Older persons are provided with the opportunities to achieve their potential by strengthening their social support systems, improving their quality of life and becoming stakeholders in their own communities. Shops include daily services that include meal programs, educational and recreational activities, case assistance , health aging and wellness programs and volunteer opportunities. Case managers and social workers are on site to connect people with assistance in obtaining access benefits from social security, medicaid, medicare food stamps and home care They also have elder abuse and crime victims assistance programs, transportation services and housing in retirement communities. My project is simply presenting health topics to an audience that are spanish speaking and aging.How can I Improve their understanding of their chronic illness and be able to make changes in their health behaviors. My project does align with the center's mission of “Helping Older Adults to help themselves.”empowering them with information on how to age gracefully without further decline of their health.
