Materiał i metody: Dziewczynka 5 letnia skierowana do Kliniki Chirurgii Dziecięcej UJ z oddziału otolaryngologii szpitala rejonowego z powodu gwałtownego pogorszenia stanu ogólnego po wykonanej kilka godzin wcześniej endoskopii przełyku. W badaniu radiologicznym stwierdzono obecność ciała obcego metalicznego, którego następnie (nie znaleziono ? uwidoczniono) podczas ezofagoskopii.
Przy przyjęciu dziewczynka w stanie ogólnym średniociężkim, z gwałtownie narastającymi objawami niewydolności krążeniowo-oddechowej i zakażenia ogólnego.
W wykonanych po przyjęciu badaniach obrazowych uwidoczniono monetę w dolnej części przełyku. Ponadto stwierdzono cechy prawostronnej odmy opłucnowej oraz odmy śródpiersiowej.
Dziecko zakwalifikowano do pilnej torakotomii. Śródoperacyjnie stwierdzono zaawansowane zmiany zapalne ze złogami włóknika i resztkami pokarmowymi w jamie opłucnej. W odcinku nadprzeponowym przełyku stwierdzono rozerwanie jego ściany na dł. 40 mm z wyczuwalną w świetle przełyku monetą. Ciało obce usunięto (2 zł). Rozerwanie zaopatrzono szwami warstwowymi. Założono drenaż opłucnowy i śródpiersiowy.
W okresie pooperacyjnym stosowano antybiotykoterapię o szerokim spectrum działania. Nie obserwowano powikłań. W 14 dobie pooperacyjnej, po wykonaniu badania kontrastowego przełyku, usunięto sondę żołądkową i rozpoczęto karmienie doustne, które dziewczynka tolerowała dobrze.

Wnioski : Rozerwanie przełyku stanowi ciężkie powikłanie procedur endoskopowych prowadzące do błyskawicznego rozwoju stanu septycznego i pogłębiającej się niewydolności krążeniowo-oddechowej.
Bezzwłoczna interwencja z agresywną antybiotykoterapią i drenażem pozwala uniknąć lawinowego narastania powikłań odpowiedzialnych za bardzo wysoką śmiertelność w tym typie urazu.
