a. Identify Material Needs:

b. Requisition Form Completion:
The requesting department completes a materials requisition form with the following information:
Date of request
Department name

Project or job number (if applicable)
c. Authorization and Approval:
The requisition form is reviewed and authorized by the department manager or authorized personnel.
d. Forwarding the Requisition:
The approved requisition form is submitted to the procurement or purchasing department for further processing.
