Tonic seizure EEG is characterized by a diffuse high-voltage slow wave followed by a generalized low-voltage fast activity. This electrical pattern is similar to the LVFA of tonic seizures, and it is thought to reflect sustained fast neuronal firing over a wide cortical area.

Lennox-Gastaut syndrome (LGS) is a severe developmental epilepsy. It is associated with intellectual disability. Affected children usually develop the disorder at about 3 to 5 years of age. LGS can be triggered by a cortical lesion.
Neuroimaging is an important part of the search for the underlying etiology. Peri-ictal SPECT, in particular, can help to identify early pattern of cerebral blood flow. Earlier patterns of blood flow in the primary cortical areas are often accompanied by a decreased amount of cerebrospinal fluid in the pons.
Tonic and myoclonic seizures are the most common types of seizures in LGS patients. Other forms include axorhizomelic tonic, which involves abduction of the arms and proximal upper limbs, and axorhizomeloic tonic, which involves flexion and extension of the extremities.
Tonic seizures are commonly characterized by a drop in tone, sudden dropping, or falling. In addition, they may involve craniofacial injury. These forms of tonic seizures occur in approximately 17% to 95% of LGS patients.
