The discussions were audiotaped and transcribed exactly. In general, doctors were reluctant to refer patients for medication or psychotherapy. They were concerned that patients might not be able to handle the additional treatment and might not attend follow-up sessions. Additionally, many GPs believed that liaison psychiatry was not being adequately supported and oversubscribed.
The findings from this research showed that GPs used pre-existing knowledge and relationships to inform their referral decisions more than the particular needs of patients. The recorded meetings showed this. CMHT leaders, Psychiatrists and GPs all spoke about the competence of GPs. This was evident in every recorded meeting.
It is important to note that the right to choose is applicable to treatment and assessments, such as neurodevelopmental diagnostic assessments. The current pandemic has caused an enormous increase in the number of people who are referred to mental health services within communities for help. This has resulted in an increase in need for psychiatry assessments and a greater workload for staff. The rapid onset of the pandemic meant that it was difficult for doctors and other healthcare professionals to keep up with the volume of referrals. This resulted in a large number of referrals that were not appropriate.
We conducted a qualitative research study within a randomised control trial to test the efficacy of different strategies for improving the quality of referrals for mental health. Interviews with GPs and community mental health team consultants (CMHT) were conducted, as well as the recording of meetings for the distribution of referrals. The study concluded that a bespoke route for referrals was a viable method to manage the workloads of doctors and CMHTs while providing patients the choice needed to receive effective treatment.
