Disorganized, tangential or circumstantial associations are thought processes that could indicate mental health conditions like schizophrenia, mania and bipolar disorder.

In addition, psychologists and neuropsychologists examine the ability of the client to pay attention and briefly hold information in their memory. This can be assessed by observing examiners, the client's self-report or brief tests like counting backwards from 100 by 7s. They also assess the client's strategies for coping and cognitive performance in everyday life, which is evaluated through direct questions and behavioral observation.

During the MSE, psychologists observe the client's facial expressions as well as body language to determine whether they appear to be withdrawn or aggressive. They also look at the client's agitation and fidgeting to determine if they are anxious or fearful.
