The Right to Choose Psychiatry uk
As a patient you have the legal right to choose where you will receive treatment. This applies to consultant-led elective care, as well as to services ordered by CCGs or NHS England.
Psychiatry UK provides ADHD, Autism, and combined assessments as part of the NHS Right to Choice pathway. This allows those with long NHS waiting lists to exercise their right to choose a provider and reduce the time they wait for an assessment.
What is the right of choice?
The right to choose psychiatry in the UK is a fundamental legal right that allows patients to decide on their healthcare. This is the case for a variety of services, including assessments and treatment. If a person is screened for a condition, such as ADHD they may opt to be referred to an alternative NHS service provider than the one in their local area. This is beneficial for many reasons, such as travel restrictions or in the event that the patient needs to see a specialist in their condition.
The legal right to choose is currently available at the time of referral from a primary care physician for first outpatient appointments for a mental or physical health condition. It is only available to providers who are clinically appropriate, and have signed a contract with NHS England or the CCG (host commissioner) that covers the service.
If a person is recommended by their local NHS Trust to receive an ADHD diagnosis it is likely that they will be referred to a local mental health service. Waiting times for a first appointment can be as long as two years. If the patient has the option, they can choose to be referred to a service that provides an online ADHD diagnosis such as Psychiatry UK which offers the fastest path to diagnosis.
This is a positive change that can improve access to mental health services in the UK. It can be frustrating to wait for an assessment before treatment. There are a variety of obstacles to improving access to psychiatry services and the delivery of services in the UK. There is a shortage of consultants and a desire to increase recruitment.
If a doctor wishes to refer a patient to the Psychiatry-UK for an online ADHD assessment as per the legal right to choice they will be required to compose a short note outlining the reason. The letter should be addressed to Psychiatry-UK and have the GP's name at the bottom (not ANP or other). The letter should also include an outline of the patient's medical history as well as an completed AQ50 (click here for template). The Psychiatry UK will contact the patient's GP to get more details.
Who is eligible to make a choice?
In England you are eligible for Right to Choose if you were referred by your GP to a consultant-led mental health team for assessment or treatment. This includes a neurodevelopmental diagnostic test for ADHD or Autism. This does not include any other NHS services, such as psychotherapy or counselling. You cannot be in the Armed Forces or in a secure setting (such as a prison, specialized training centre or a young offenders' institution or immigration removal centre).
To exercise your right to choose, you must have been referred by a GP to an NHS provider that is commissioned to provide the service. The service offered must be clinically appropriate to your requirements. Psychiatry UK offers ADHD assessments to children and adults under the age of 19 through Right to Choose. These appointments are delivered in person. If you prefer an online appointment, they are able to provide a private ADHD assessment for adults and children. They also provide titration services for medication following an assessment, but this is not available through RTC.
It is vital that you let your GP know that you'd like to exercise your right to select. You can download a referral letter from most Right to Choose providers and give it to your GP. The letter provides the information they require to make an appointment. Click here to download a copy of the template from Psychiatry UK.
How do I exercise my rights of choice?
If your GP recommends you for special mental health treatment you have the legal right to choose the healthcare provider. You won't be penalized by the NHS if you choose to visit private hospitals than if a GP referred you. You are entitled to choose a service or provider with a NHS Standard Contract with any CCG, NHS England or any CCG for the service you require (this includes a variety of hospitals). You cannot use your right to to avail cancer treatment services that have a two-week waiting period, maternity services, or mental health services provided by the armed forces.
In a survey of future psychiatrists conducted between one, three and five years following medical school, 4% of respondents opted for psychiatry as their first choice in a career. When comparing this number to doctors who opted for other careers in clinical, it was similar to the 3-4% of Americans who chose psychotherapy. The figures were higher for women than for men, indicating the trend of women deciding to become psychiatrists than men.
Psychiatrists' choice of their career in psychiatry is influenced by a range of factors. Of these, 'enthusiasm/commitment: what I really want to do' was the factor rated as having most influence by over half of respondents in year one. 'Student experience of the subject','self-appraisal of own skills' and 'inclinations before medical school' were also cited as having a significant effect on choice.
For those who suffer from ADHD their doctor may make use of the NHS's Right to Choose to refer them for an independent assessment with Psychiatry-UK. A specialist ADHD specialist will provide an assessment and medication titration. Here, GPs can get more information, including an email they can send to their patients.
You can only exercise your Right to Choose for other conditions, like OCD If your GP suggests an assessment by a mental health specialist and you've been released from the service. Your doctor may refer you to Psychiatry UK from the specialist mental health service. We can then provide an assessment and titration of any medication prescribed.
What if I do not wish to exercise my right to choose?
You aren't required to make use of your right to choose in the event that you do not want to. If you decide to exercise your right to choose, it is important to be aware of the implications. If you decide not to exercise your right to choose, it will have an impact on the speed at which you receive an appointment and the type of service you receive. It will also affect the options you have in the future should you need to be referred again.
It is important to tell your GP immediately when you decide to not use this right. They'll direct you to a clinic that's best for your requirements. It's possible that you'll have to wait longer for an appointment, but it's worth waiting when you think it's the right thing to do.
You can ask your GP if Psychiatry UK offers an online ADHD assessment. The website offers details on how you can conduct this assessment as well as the form you can download to present to your GP. This service is available to adults and children. However they do not provide titrations of medication via the NHS following an assessment. They currently offer an assessment for patients in Kent and Medway as well as Isle of Wight and North East Hampshire.
Psychiatry-UK is a non-profit organization that offers free secure, confidential, online consultations with qualified psychiatrists. These consultations can be booked through a secure online booking system. These consultations are recorded and stored on a medical database, and the information is used by a team of specially trained clinicians to monitor patient health. The clinicians are also supervised by senior mental health professionals to ensure that the treatment is provided to the highest standards.
The authors would like thank the nine participants for their time and efforts. They were selected at different stages of their medical training and from a wide range of backgrounds. From second year at BSMS and just about to complete their fifth year of medical school. They were interviewed via videoconference or email and asked to share their experiences while choosing a career as psychiatrist.
