Psychiatry and the Right to Choose
NHS patients are legally entitled to choose their initial outpatient appointment. Some GPs don't understand how they can work. Siobhan Rourke discovered this when she attempted to get an ADHD assessment via Right To Choose.

Psychiatry UK has been registered as an organization that offers Right to Choose assessments
Right to Choose is a legal right given to patients by the NHS in England to be able to choose their own clinical team for non-urgent consultation with a consultant. This includes the field of psychiatry. However, it doesn't apply to services that are commissioned by local authorities, such as the maternity services or community mental health services.

The discussions were audiotaped and transcribed in full. In general, doctors are reluctant to refer patients to a psychotherapist or for medication. They were worried that the patients wouldn't be able to handle the additional treatment, and that they might not attend the follow-up appointments. Many GPs were also of the opinion that liaison psychiatry had inadequate resources and was oversubscribed.
The findings of this study revealed that doctors rely more on pre-existing relationships and their knowledge to make referral decisions, rather than on the specific needs of a patient. This was evident in all the recorded meetings. CMHT leaders, Psychiatrists, and GPs all expressed their opinions on the competence of GPs. This was evident in every taped meeting.
It is crucial to remember that the right of choice applies to both treatments as well as assessments including neurodevelopmental diagnostic assessments. We conducted a qualitative study that was incorporated into an uncontrolled trial of randomisation to explore the efficacy of a variety of strategies aimed to improve the quality of referrals for mental health. Interviews with GPs and community mental health team consultants (CMHT) were conducted, as was the recording of meetings to aid in allocation of referrals. The study found that a customized pathway for referrals was a successful method of reducing the workload of GPs and CMHTs while giving patients the choice needed to receive effective treatment.
