Mental Health Nursing - Assessing Mental Health
Mental health assessments usually involve a physical examination, to rule out physical ailments. This can include urine tests, blood testing, or even brain scans.

Other important aspects to consider are whether the patient's thoughts are mood compatible or not in line with their surroundings. A person suffering from depression could hear voices of anger, while a person suffering from schizophrenia might experience a hallucination that a soothing voice is speaking. Ask the patient if they've had any suicidal or homicidal ideas. Although they are not common however, they can be life threatening and should be taken into consideration in every mental health assessment.
Observation
