Psychiatry and the Right to Choose
NHS patients have a legal right to pick their first outpatient appointment. However there are some GPs aren't sure how they work. Siobhan Rourke discovered this when she tried to obtain an ADHD assessment via Right To Choose.
Psychiatry UK offers online consultations via video conference with psychiatrists for fee-paying and some NHS patients. The service offers assessments as well as prescriptions.

Right to Choose is a legal right given to patients by the NHS in England to be able to choose their own medical team for non-urgent, consultant-led treatment. This includes the field of psychiatry. If you are referred by your doctor to an NHS hospital or service for mental health you have the right to select the location you want to go. This right to choose applies only to those facilities that have an NHS standard contract with your CCG or with NHS England for the service you require. This includes psychiatry.
The field of psychiatry studies mental illness, its causes, diagnosis and treatment. It involves the use of medication and other methods such as psychotherapy. It is a highly-specialized area that requires extensive knowledge and expertise. Psychiatrists work with patients, their families, and other healthcare professionals to assist patients overcome the symptoms of mental illness.
Clinical commissioning groups (CCGs) are the main providers of NHS services across England. These organisations are responsible to oversee the commissioning of NHS services within their local area. They can also provide the titration of ADHD medication. They are unable to provide a full ADHD diagnosis in the 90-minute appointment. We accept referrals from GPs
GPs are legally able to refer patients for specialist treatment under the NHS, including psychotherapy and medication, under Right to Choose. In order to know how GPs decide whom to refer to to, the TAG team carried out a series of focus groups with doctors and MHPs in eight different GP surgeries. The discussions were audiotaped and then transcribed verbatim. In psychiatry-uk reviews , doctors were reluctant to refer patients to therapy or medication. They were worried that the patients would not be able to cope with the additional treatment, and they might not attend the follow-up appointments. Additionally, many GPs believed that liaison psychiatry was not being adequately supported and oversubscribed.
The findings from this research showed that GPs used pre-existing knowledge and relationships to inform their referral decisions more than the needs of a patient. The recordings of all meetings demonstrated this. CMHT leaders, Psychiatrists and GPs all commented on the level of competence of GPs. This was evident in every taped meeting.
It is important to remember that the right of choice applies to both treatments as well as assessments, including neurodevelopmental diagnostic assessment. The current pandemic has led to a huge increase in the number of patients directed to mental health teams in the community to receive support. This has resulted in an increased demand for psychiatry assessments and an increased workload for staff. It was challenging for GPs to keep pace with referrals due to the rapid onset of the pandemic. This led to a large number of inappropriate referrals.
We conducted a qualitative study that was incorporated into an experimental study that used a randomised control to test the efficacy of different strategies for improving the quality of referrals for mental health. Interviews with GPs and community mental health team consultants (CMHT) were conducted, as was the recording of meetings to aid in allocation of referrals. The study found that a customized pathway for referrals was a viable method of managing the workloads of doctors and CMHTs while providing patients with the option to receive treatment effectively.
