The FDA has identified Avanos Medical's recall of the Cortrak*2 user access system. 629 devices manufactured between 2016 to 2022 were recalled in the U.S., beginning on the 21st of March.
Cortrak*2 assists clinicians in placing medical feeding tubes inside the stomachs or small bowels of patients who require to be fed through the tube.
This recall was initiated in response to reports of injuries and even death among patients after mistakes in the placement of gastric or nasocentric tubes. According to Avanos’ safety communication, there were 60 deaths and 23 injuries resulting from this issue. The adverse events mentioned included perforation, respiratory failure, and pneumonia, in addition to the development of pleural effusion.
Avanos Medical feeding tube This recall will be utilized to modify Cortrak*2's guidelines regarding use, intended usage and also advise users to confirm tube placement in accordance with institutional guidelines prior to using the tube to supply nutrients.
Avanos Mediacal Cortrak 2 Avanos Mediacal Cortrak 2 Clinicians were advised to attach the corrective field notice about the issue in their operating manual and to return the acknowledgment form that was included in the notice to Avanos. The company plans to offer users updated labeling, including confirmation of the placement of tubes according to institutional policies before the use.
