Avanos Medical Cortrak*2 Ental Access System was designed to allow medical professionals who are trained to insert medical feeding tubes in the stomachs or small bowels of patients who require nutritional support. 23 deaths have been reported to date due to feeding tubes in 2015.
There have been numerous reports of injuries as well as patient deaths that have occurred when nasogastric tubes were misplaced while this device is used to aid in their positioning.

Avanos Medical The incorrect placement of a Nasogastric or Nasoenteric tube could cause harm to the vocal chords, lungs or trachea. This could result in grave injury and possibly death.

Avanos Medical Respiratory failure, pneumothorax, perforation (a hole, in the stomach or wall) and pneumonia were all reported.
On March 21 2022, Avanos Medical issued a field correction notification to its customers who used these devices between January 2021 and January 2022. Avanos Medical The following guidelines were included in the notice.
Verify the location of Nasogastric tubes and the nasoenteric tube according to institution policies.
Attach the corrective notice in the field to the operator's handbook.
