The FDA has confirmed Avanos Medical's recall of its Cortrak*2 oral access device. Beginning on March 21 the 21st of March, 2016, 629 devices were recalls in the U.S.
Cortrak*2 assists clinicians in placing medical feed tubes into patients that require nutrition via the tube.

Clinicians were urged to attach the corrective notice for field use concerning the issue to their operating manual and return the acknowledgement form that was included with the notification to Avanos.
