Avanos Medical Cortrak 2 Entry Access System is created to assist trained health professionals in inserting medical feed tubes in the stomach or small bowel of patients who are requiring nutrition. There have been 23 deaths reported up to date due to the feeding tubes since 2015.
Avanos Medical Many reports have revealed that patients suffered trauma or even died when their nasogastric tubes or nasoenteric ones were not properly placed. It's despite the fact that this device was utilized to aid in their placement.
This recall was utilized to create revisions to the device’s labels and instructions for use and purposes. The updated instructions instruct users to confirm tube placement according to the protocols of their institution before using the tube to deliver nutrition.
If a nasogastric tube is not inserted correctly it is possible for patients to suffer injury to the vocal cords, the lungs, or trachea, all which can lead to serious harm or even death.

Avanos Medical Avanos Medical issued a correction notice to customers who had used these devices between January 2021 through January 2022 on the 21st of March, 2022. The notice provided the following information:
Verify the placement of nasogastric/nasoenteric in accordance with institutional policies
Add the notice of correction for field concerning the problem to the operator's handbook.
