Basically there is a girl in malignant who have a imaginary friend in childhood and when she grew up she suddenly start having some wierd dreams about people's death and she sees all of them dying by someone like she is sitting in that particular place , but these turn out to be real incidents which she saw in her dream , she sees exactly how those people died and told the police about it but the police didn't believed her and thought she might be the criminal so they arrest her there we get to know that she actually was attached with her twin on the back of her head which was hidden by some surgeries i guess but now it came out again so it's like attached on the back of her head so you can say he was the one killing every person means that the girl's body was killing every person but while the twin was killing the girl went unconscious as they both shared the same brain they had control on each other in this manner it was girl's body but the twins intentions who killed others , and at the end the girl finally finds out how she can control her twin and then it's a happy ending so the imaginary friend turned out to be real
