A tonic clonic seizure is a type of epileptic attack that affects the tonic phase of muscle activity. It is important to understand that tonic clonic seizures can be treated. Some treatments include nerve stimulation, diet and surgery.
Nocturnal frontal lobe epilepsy (NFLE) is a peculiar form of focal epilepsy that occurs almost exclusively during sleep. Patients with NFLE exhibit bizarre motor behavior and sustained asymmetric dystonic posture.
In order to define the diagnostic criteria for NFLE, a detailed clinical and neuro-physiological evaluation was carried out. Four patients presented with nonlateralizing generalized tonic-clonic seizures in sleep. Each patient underwent an MRI of the brain. The findings showed focal atrophy of the right hippocampus.
Two patients had significant head injuries in early adulthood. One patient's EEG showed diffused 3.5-4 Hz multifocal spike-slow waves during a tonic-clonic seizure.
Another patient's EEG recorded no seizures. However, she did have episodes of paroxysmal arousals. During these episodes, she had olfactory hallucinations. She also had episodes of vertigo and anterograde amnesia.
A third patient had a history of generalized tonic seizure at 3 months of age. His parents were non-consanguineous. He had a normal brain MRI and EEG.
Four other patients presented with a history of nocturnal tonic clonic seizures. They had been followed for years. Their symptoms included:
Patients in this study had a median age of 11 y. The male/female ratio was 7/3.
