Nocturnal tonic seizures occur during sleep. They are usually short and brief, lasting only a few seconds. Seizures may involve the head and body. Symptoms include headache, confusion, and soreness.
During the ictal EEG, generalized spike and polyspike-wave discharges are common. These discharges may be in a single burst or in clusters.
Ictal EEGs with repetitive spike-waves are indicative of tonic axial seizures. This type of seizure is usually associated with complex epilepsies. In addition, tonic axial seizures are associated with drop attacks in the awake state.
Other types of nocturnal tonic seizures include those associated with Lennox-Gastaut syndrome. Children with this condition often exhibit cognitive impairment. The disease is also associated with a family history of SCN1A mutations. Genetic testing is available for these genes.
