Avanos Medical has recalled the Cortrak*2 Enteral Access System due to reports of injuries and deaths resulting from nasoenteric and nasogastric feeding tubes were installed incorrectly.


Patients may suffer serious injuries or even death if a nasogastric tube or nasoenteric tub is wrongly inserted. Avanos Medical issued a announcement to the healthcare providers to verify the placement of the NG/NI tube per institutional procedure. Fox news announced.

Avanos Medical's recall communications stated the deaths and injuries caused by the misplacement of the tubes feeding into the enteral lining while using the CORTRAK* 2 Enteral Access System from 2015.

Baxter has received 51 reports about serious injuries and three cases of patient deaths which could be linked to this concern over the last five years.

Following numerous complaints of security alarms not working and malfunctions in safety alarms, the medical device manufacturer Baxter has issued a recall on the device. If there was an upstream obstruction the alarm could not trigger on the pumps. Warnings were made that these products could result in adverse health consequences and may even lead to death.

There have been three accidents and 2 deaths attributed with the use of this device.

Three injuries and two deaths resulted from the NIMCONTACT Reinforced EMG Endeotracheal Tube and NIM Standard Reinforced EMG Endeotracheal Tube , which are used to check laryngeal and head nerves.
