Avanos Medical, a U.S. feeding tube placement Food and Drug Administration (FDA) company, is recalling Cortrak*2 enteral access system because of misplacement of nasoenteric tubes or nasogastric tubs which could have resulted in injuries or even death of patients. The recall includes 629 devices currently distributed in the United States.
Avanos marketing its Cortrak*2 Endural Access System is known for its screen-based visualizations of stomachs of patients and small bowels to aid in the placement of medical feed tubes. If a tube is placed improperly it is possible for patients to suffer injury to their vocal cords, lungs, or the trachea. The recall notification from Avanos Medical states that, since 2015 there has been 60 deaths and 23 injuries related to the incorrect placement of nasogastric feed tube when using the CORTRAK*2 enteral access system. Pulmonary failure, pneumothorax (collapsed lungs) and perforation (a hole in the esophagus or the bowel wall) as well as pneumonia and excess fluid that is accumulating between the lung and chest cavity (the chest cavity as well as the lungs) are just a few of the traumas.
Avanos Medical described a plan for users to receive updated labels to ensure proper positioning of the nasogastric tubes and nasoenteric pipe prior to use.

The FDA advised healthcare professionals and parents of the risk of strangulation for children who use feeding tubes. Two deaths resulting from strangulation of children was reported in 2021 as a result of tubes that were wrapped around a patient’s neck.
