合約範本以中文或英文為限，但為便利後續案件套用，請將計畫名稱、計畫主持人姓名及預算金額等留白並以顏色底色標註。 合約範本議定時，可同時約定範本之有效期限，惟若廠商或本院更新合約相關表單或說明時，需同步更新已議定之合約範本。 承辦人收到變更申請後會排入審查流程，審查後約14個工作天(可能視案件排程狀況延長或縮短審查天數)，本中心將以email回覆合約條文修正建議，請修改後以"追蹤修訂"、"回覆所有人"方式，附加檔案於email寄回。 (二) 臨床試驗自費項目由委託者給付，應勾選需記帳代碼，以做為登記計畫內需支付的檢查或檢驗等相關費用。 ※本院認定之合約範本僅限與本院簽訂長期制式範本或主約，若套用曾經簽署過某計畫案之合約，視為新約收費及審查。 法律用語，以避免大家在理解合同時產生不應有的歧義。
