我們可以把植牙手術比喻成蓋房子，補骨手術等同於打地基，如果在沙地和坡地上蓋房子，容易造成土壤流失和地基露出，相信大家都能夠認同，地基應該要建立在穩固且堅硬的地上。 這也是很多病人的疑問，最主要是多數患者在失去牙齒後，並未立即處理治療，顎骨隨著時間會逐漸萎縮，其寬度和高度都會逐漸縮水，當時間拉長，其缺牙處的顎骨呈現尖凸樣，是相當不利於植牙的狀態。 對於補骨比較有概念之後，另外，在植牙手術前需要評估骨頭量之外，補皮肉也是植牙成敗的關鍵。
總之，在確立失智症診斷後，要記得健康的口腔環境以及功能健全的牙齒對於將來失智症照護，是否能事半功倍是很重要的因素。 在至牙科檢查後，若評估患者有全口贗復（製作假牙）的需求，最好在失智症極早期或輕度的時候便能完成，此時絕大部分的治療都可以在患者原本家庭所熟悉的牙科診所完成，否則到了中重度之後，可能就要借助「特殊需求者牙科」（註）的幫忙了。 條件上是否實際可行，仍要回歸到治療計畫的設定上 ; 排蠟牙在CT上是否有可能達成? 筆者認為，臨床上我們一直在處理的是兩件事，"發炎"的控制與"力量"的控制; 患者掉牙的因素多半是蛀牙與牙周病, 因此口腔衛教的指導與動機的不斷提醒，一向是筆者的診所非常重視的 ! 有了乾淨的環境後，若牙齒的受力沒有獲得合理的分配，那就像是"過勞"的工作者，久了一定還是會出問題。 上顎後牙區因為患者較不好清潔，大臼齒本身又有三個牙根分叉或是凹陷等解剖構造的限制因素，所以常有缺牙的問題發生；隨著牙齒的缺失，骨頭伴隨發生的水平與垂直骨萎縮可能高達50%與20%之多！ 這些不利情形，都會造成後續想要以植牙重建的困難(註1)；幸好，藉由鼻竇增高術可以有效達成骨嵴增加的需求。
您可以在普通牙科診所進行局部麻醉，直接拔牙。 一般使用牙槽部浸潤麻醉，或對有關的神經干做阻滯麻醉。
在這種情況下，拔除是在您入睡後由醫院的牙科醫生進行的。 如果剛進食了pH值比較低的食物或者喝了碳酸飲料，那30分鐘內最好不要刷牙。 因為牙釉質受到酸的腐蝕後會變得脆弱，這個時候刷牙確實會傷害牙釉質。
