- ผู้ป่วยมีอาการ heartburn มานาน แสดงว่ามีการหลั่งกรดในกระเพาะมากเกินนานพอสมควรที่จะทำให้ lower esophageal sphinctor (LES) ทำงานผิดปกติ ทำให้มีกรดไหลย้อนขึ้นไปที่ esophagus ได้
- การสร้างHCL ในกระเพาะมากเกินจนทำให้ defense mechanism เสียไป จึงมีอันตรายต่อ gastric mucosa จนเป็น ulcer และ bleeding ได้
- ผู้ป่วยรายนี้มีถ่ายดำ (melena) แสดงว่ามี hemorrhage at upper GI tract เนื่องจากเลือดจะทำปฏิกิริยากับกรดในกระเพาะ (Hb + acid = acid - hematin) กลายเป็นสีดำเหนียวคล้ายยางมะตอย มีกลิ่นเหม็นเฉพาะ (ถ้าเป็นเลือดที่ออกจาก lower GI tract จะเป็นสีแดง เพราะไม่โดนกรด)
