Egaeus is a very smart and a very unhappy man who grows up in a large and depressing mansions with his cousin, Berenice. Egaeus suffers from an obsessive disease, monomania which causes him to focus on the smallest things for a very long time. Berenice was a very happy and beautiful woman but she also suffers from a disease in which causes her to become very ill and very hideous. Egaeus and Berenice were going to get married but Berenice gets sick and one afternoon Egaeus see’s Berenice while he’s sitting in the library and she smiles at him and he notices her teeth, his disorder makes him become obsessed with them. A servant tells Egaeus that Berenice has died and she is to be buried. Egaeus snaps back to reality and notices a small box which once belonged to the family physician but he had no idea why it was there. Another servant approaches Egaeus and tells him that Berenices grave had been violated and Berenice was still alive. Egaeus notices his clothes and his hands were covered in mud and blood, he then picks up the mysterious box and he’s so nervous that he drops the box and it shatters revealing dentistry tools and 32 white and ivory substances scattered across the floor.
