1) Introduction: I tell if I think the game is good, okay, mediocre, bad, etc. And tell some history about it if I need to mention anything like that.
2) Game play: I just tell some of the features for example, if there are side quests, multiplayer, etc. And say if they did a good job of it or not.
3) Controls: I tell if I have to push a button more than once to make something happen, or not.
4) Challenge level: I tell how challenging the game is, and if it is challenging for the right reasons or not.
5) How fun it is: I tell if I have fun with the game or not, and say why.
6) Conclusion: I tell what I thought of the game overall, and give it a rating out of 10.
7) Revision: Check your wording, spelling (only needed if it is a written review, might be helpful for a script if you are doing a video), etc.
8) Recording (videos only): If it is a console game, get an emulator if you own the game or use a capture card (good idea if you have one, better than emulators) if it's a PC game, just record it on your computer. Voice over is the tricky part, if you have a small microphone, try to speak loud and clearly, make sure there is no distractions, etc.
9) Editing (videos only): Cut out anything you don't want, add the video and audio and then publish.
10) Publishing: Make sure you are happy with your review, and if it is written just publish it. If it is a video, upload it to a video site (eg. youtube.com) and then you are done.
