Notes

Patient complained of anxiety and nerve pain. States that his anxiety affects his work by making it difficult to concentrate all the time. Anxiety attacks began 2 years ago but stopped when he started taking Xanax. Currently taking Xanax 1mg PO bid, which helps with his anxiety. Anxiety comes back when his Xanax prescription runs out. Tried taking Elavil before but stopped due to side effect of nerve pain, which has not stopped since discontinuing the medication. Complains of numbness and tingling to his hands and fatigue to his arms, face especially upon working out. Has not been able to exercise recently due to anxiety. States that he feels pressure to ears when lifting weights and that he gets exhausted quickly when exercising. Denies family history of anxiety or depression.
Plan: start taking bupropion, smoking cessation counseling, start multivitamin. Return for follow up visit in 1 month.
