If You Didn't Receive The Form Or Simply Did Not Return The Form, you can go ethir go with Your Primary Doctor and request it, or we can help you get it done Over The Phone.
If You Choose To Do This With your Primary Doctor, You Have To Go Personally to Fill Up The Form With Him, Now Of Course This Has A Due Date!!
Now If You Choose To Do This Over The Phone I Just need To Verify The Information I Already Have On System.

(What Happens If I Dont Fill Up This Form?)
Your Current Medicare Claim Number Is Not Going To Be Valid After April Of This Current Year Due To the New Security Protocol, If You Do Not Generate Your New Patient Number, We Will Not Have A Way To Provide You Benefits Or Identify You As A Patient Here At Medicare, So Basically Your Coverage Will Be Interupted After April, And You Will Be Called In To Your Nearest Medicare Office During The Month Of August To Be Able To Generate This New Number.

(Why Do I Need To Fill Up The Form)
This Process Is Done Due To A New Security Protocol That Was Requested By The Social Security Administration Since During The Past Year We Received Hundreds Of Reports Of Lost And Stolen Cards Which Contained The Social Security, And Basically If You Loose Your Social Security Number You Loose Your Identity, So For That Purpose You Will Be Provided A New 11 Digit Numeration, A Combination Of Letters And Numbers That Have Nothing To Do With Your Identity.
