If you want to be reimbursed for SHPE National Conference registration there are a couple things you need to complete:
FIRST: Pay $10 Chapter Membership Due for 2016-2017
You must pay me in person, it would be best to pay me at the general meetings if you can not make those. Please email me with available days and times to meet
You can also contact me if you are unsure if you already paid
SECOND: You must fill out the Online Membership Application 2016-2017
Fill out this form
THIRD: You must fill out the PRF form attached to this email with personal info and mailing address
Download one PRF based on registration day you paid for:
Wednesday -> "WED_SHPE National Reimburse..."
Thursday -> "THURS_SHPE National Reimburse..."
Program's Only -> "Prgms Only_SHPE National Reimburse..."
Replace "Juan Smith's" information with yours (basically fill out highlighted section)
Name the PRF with your First & Last Name
THEN email the PRF back to me for processing
Note: For the mailing address, you can choose your home or college address. If you don't pick up your check in 2 weeks, the address you put is the one the check will be sent to.
Note: If you modify the PRF outside the highlighted sections, your reimbursement will be dropped
FOURTH: Print confirmation email receipt and turn in to Nidia (me) or Charly
The printed receipt must show your name right below the bar code in order to be valid for ASI purposes
Receipt must show Wednesday, Thursday, or Programs Only registration
Note: you can search your email for "shpe 2016 registration" for easy retrieval
I repeat these 4 steps should be completed if you wish to receive a reimbursement from our chapter upon your return from conference.
