Glikolizde bir molekül glikoz (C6H12O6) parçalanarak pirüvata (C3H6O3) dönüşür. Bu süreçte glikozdan iki gliseraldehit-3-fosfat (G3P) molekülü elde edilir. Bunun ardından bir nikotinamid adenin dinükleotit (NAD+) molekülü G3P'den bir hidrojen atomu çıkararak G3P'yi 3-bifosfogliserata dönüştürür, NAD+'yı da NADH'ye.
Oksijen olduğu zaman NADH'deki hidrojen başka moleküllere aktarılarak sonunda oksijenle reaksiyona girer ve su (H2O) meydana getirir (ve bu yolla hücreye enerji sağlayan ATP üretilir). Böylece NADH tekrar NAD+'ya dönüşür ve yukarıda belirtilen tepkimede tekrar kullanılabilir. Yeterince oksijen olmadığı durumlarda hücredeki tüm NAD+, NADH'ye dönüşür ve ondan sonra G3P daha fazla 3-bifosfogliserata dönüşemez. NADH'deki hidrojeni alacak başka bir bileşik olmazsa daha fazla ATP üretilemez. Fermantasyon böylesi bir bileşik sağlar.
Etanol fermantasyonunda pirüvattaki iyonlaşmış karboksil grubu (COO-) ondan kopup bir molekül karbon dioksit olarak ortama salınır. Arta kalan molekül, asetaldehit (C2H4O), NADH'deki hidrojeni alarak ortamda bulunmayan oksijenin işlevini görür. Bu hidrojen, glikolizin daha evvelki bir aşamasında açığa çıkan bir H+ iyonu ile beraber, asetaldehide eklenir ve etanol (C2H6O) meydana gelir.
